Chronic disease management

Structured care plans are available to help manage long-term conditions such as diabetes, cardiovascular disease, and arthritis. we work in partnership with allied health providers and specialists to deliver coordinated care.

There are two types of care plans that can be prepared by a General Practitioner (GP) under the Chronic Disease Management (CDM) program:

  • GP Management Plan (GPMP)
  • Team Care Arrangements (TCAs)

If you have both a GPMP and TCAs in place, you may be eligible for Medicare rebates for certain allied health services. These services must be directly related to the management of your chronic condition, and eligibility is assessed by your GP.

A chronic medical condition is one that has been (or is likely to be) present for six months or more – examples include asthma, cancer, heart disease, diabetes, arthritis, and stroke. There is no set list of eligible conditions; however, these plans are designed to support patients who require a structured, team-based approach to ongoing care.

A GP Management Plan (GPMP) provides an organised strategy for managing chronic illness.
A Team Care Arrangement (TCA) supports patients who need care from multiple providers by ensuring collaboration between your GP and other health or care professionals.

Once a plan is in place, it should be reviewed regularly. These reviews ensure that your health goals are being met and allow for any necessary changes to your care plan.
